Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 and all

PLEASE READ CAREFULLY AND ENTIRELY BEFORE SIGNING

I understand that COVID-19 (Coronavirus) is very contagious and is believed to be spread mainly from person to
person contact, through the air, and from contaminated surfaces. There are a range of symptoms which include
but not limited to fever, shortness of breath, fatigue, loss of taste and smell, Nausea and vomiting. These
symptoms can be mild or severe, sometimes resulting in death. For additional information visit

I acknowledge that a person does not have to be showing signs of illness in order to spread this virus. I understand
that the risk of person to person spread of the virus is increased by close physical contact. As a result of this States
and Federal governments along with Federal and State health organizations have recommended social distancing,
limited congregating of people and requiring the use of face coverings. These measures are put in place to reduce
the spread but there is still no guarantee you will not become infected.
By signing this agreement, I acknowledge the nature of COVID-19 and I fully and completely understand the risks
involved. I understand I am not required to participate in any SPEED CAPITAL COMMISSION (DBA SPEED CAPITAL)
activity. I voluntarily assume the risk to participate and accept sole responsibility for any personal injury, illness,
disability, damage, loss, claim, Liability or death as a result of participating in athletic events presented by SPEED
CAPITAL COMMISSION (DBA SPEED CAPITAL).

I agree to forever release and waive any liability of the SPEED CAPITAL COMMISSION (DBA SPEED CAPITAL). I
hereby release, covenant not to sue, discharge, and hold no fault to SPEED CAPITAL COMMISSION (DBA SPEED
CAPITAL) employees or Facility used for events, representatives, and agents of and from the claims, including
actions, all liabilities, damages, cost or expenses of any kind that may arise as a result of my participation in any
SPEED CAPITAL COMMISSION (DBA SPEED CAPITAL) event. I understand and agree that this release includes any
claims based on omissions, negligence, or actions of SPEED CAPITAL COMMISSION (DBA SPEED CAPITAL), its
agents, representatives, whether a COVID-19 infection occurs before, during, or after attending any SPEED
CAPITAL Event.
THIS AGREEMENT INCLUDES A WAIVER AND RELEASE OF LIABILITY. I HAVE READ THIS AGREEMENT. I
ACKNOWLEDGE THAT I HAVE READ THE FOREGOING AND UNDERSTAND THE CONTENTS OF THE WAIVER. I
ACKNOWLEDGE THE RISKS ASSOCIATED WITH PARTICIPATING IN THE EVENT AND THE POSSIBLE CONTRACTION OF
COVID-19, I VOLUNTARILY SIGN THIS FORM.
